The first-generation drugs are effective against almost all gram-positive bacteria, except Enterococcus, and their activity includes β-lactamase-positive staphylococcus. They also have greater activity against members of the Enterobacteriaceae than penicillin G. Compared to others in this group, cefazolin has the greatest gram-negative activity (Petersen and Rosin, 1995), and it has been grouped with the second-generation drugs in some references based on this activity (Williams et al., 2001). Gram-negative bacteria may develop resistance by inhibiting penetration and by producing β-lactamase enzymes.
